If your address has changed since the last time you registered with us, or you are new to CYO and coming in form another (non-Alliance) hockey center, the Alliance Hockey Association requires a Residential Move Form to be filled in.  During the online registration process, you will be asked for your address.  Even if this information is updated online during registration, Alliance will require this form to be filled in an physically signed before they will authorize your child to play hockey.  This is an requirement of Hockey Canada.

Local Residential Move

If you have moved within the city boundary or are still part of the same association please print out RESIDENTIAL MOVE FORM.   Fill it in and bring it to the CYO office, or scan the completed form and send it to [email protected]

Residential Move within Ontario

If you are coming to CYO from another center in Ontario, you will need to complete the 2018-19 OHF RESIDENTIAL MOVE FORM. You must contact the Minor Hockey Association that you are LEAVING first to get the form and submit the appropriate supporting documents. Submit the form and supporting documents to the registrar of the organization you left and they will forward the information with approval from the governing hockey association. Once approved the documents will be sent to the CYO Registrar.

Interbranch Transfer

If you are coming to CYO from outside of Ontario, you will need to complete an Interbranch Transfer Form. Fill it in and bring it to the CYO office, or scan the completed form and send it to[email protected]
